Medicaid Waiver Case Manager (Full-time)
March 11, 2025 OPELIKA, AL:
Under direction of the Area Agency on Aging Director, enrolls clients, implements and monitors care plans; maintains accurate and complete record of clients’ case file, case management activities, and service billing for the Home and Community Based Medicaid Waiver. Conducts monthly face-to-face visit with each client. Performs other duties as assigned. Assists with other Aging programs as needed, to include SHIP, ADRC, and Senior Center activities.
REQUIREMENTS: Bachelor’s degree in social work preferred or closely related field. Current State of Alabama Case Manager preferred or the ability to earn the State of Alabama Case Manager certification after hire. Willing to submit to pre-employment background investigation.
